The systems interact in ways I still don't fully understand after all this time, which is impressive and occasionally frustrating.

The pathing can get janky when your colony grows past a certain size, colonists queue up at doorways like it's rush hour. The mood and health systems more than make up for it though, the amount of interlocking detail in how an injury or a bad memory ripples through a pawn's behavior is still something few games attempt seriously.

I get why people love the story generator, but the combat AI does some genuinely dumb things once your colony gets big enough. Raids feel more like a math problem than a threat after a certain point, you just build the right killbox and it's over. The narrator system is clever, sure, but I don't think it earns the spot this high when so much of the late game reduces to spreadsheets.

It's one of the few games where every run actually feels different from the last one, and that alone justifies its ranking for me.

Different from the last one, that's exactly why I keep coming back after the runs that ended badly on day twelve or so.
